Therapy with Zack Policies

Privacy Policy:

Limits to Confidentiality:
What you share with your psychotherapist is private and will be kept confidential. However, there are some exceptions to the psychotherapist's duty to maintain confidentiality listed below:

  • If the psychotherapist believes you or another person is at risk of imminent harm.

  • If the psychotherapist learns that a child, elderly, or dependent person is or may be at risk of abuse, neglect, or in need of protection.

  • If there is a case of sexual abuse by a regulated health professional.

  • For the purpose of complying with a legal order such as a subpoena, or if the disclosure is authorized by law.

When information is disclosed, only that portion that is relevant to the request is shared.

Information Security:

  • Clinical records encompass a client profile (personal information provided by the client at the outset of the therapeutic relationship) and corresponding treatment records.

  • Therapy With Zack uses Owl Practice as an all-in-one platform for maintaining and housing client records and billing. Owl Practice was designed to be compliant with all Canadian federal and provincial privacy laws, including the Personal Information Protection and Electronic Documents Act (PIPEDA), and the Personal Health Information Protection Act (PHIPA).

  • Where a Registered Psychotherapist is the custodian of the clinical record, they are required to retain the record for at least 10 years from the date of the last interaction with the client, or for 10 years from the client’s 18th birthday, whichever is later. For example, if a child is age 7 at the time of the last interaction, the record would be kept until they are 28 years of age.

Cancellation Policy:

We require at least 24 hours' notice by emailing therapywithzack@gmail.com to cancel or reschedule an appointment.  The full session fee will be charged for missed appointments or cancellations made with less than 24 hours’ notice.
